Please write an essay in 100 words or less stating your goals in life, how higher education enters into these goals, and why you feel that you are deserving of the Polish Heritage Scholarship.

Please attach any information that you feel is important to the application, such as any unusual circumstance pertaining to financial need.

Attach your letter of recommendation from a person other than a relative to the application.

Attach a description/statement of Polish heritage, signed, and dated.

I give the Financial Aid Office permission to release any relevant information that they have for purposes of processing the scholarship to the Polish Heritage Society. This will not be limited to my academic and financial records.
